I'd like to have seen Oakley kill a gungeoneer. I always thought that when he said he would crush a dungeoneer he would probably drop the large branch on them.
What if someone got all the questions wrong at the weeping doors.
How about someone falling off Smirkanauf.

Here's one for you... how it would have looked had the Giant actually sneezed (Series 1, episode 2, I think)

I would have liked either a hob goblin or cavernwights to eat some1 alive ooooh naasty or mayb even a skeletron!!

That great big giant actually sneezing over those dungeoneers (can't think of a quest from memory) in the earlier series would have been a bit of nasty way to go.
Mogdred grabbing a dungeonner with his bare hands and throwing them into a chasm would have been entertaining too but the sadism there might have been too much for kids TV.
A series of ledges appears and one could secretly be a springboard. The dungeoneer manages to land a foot on the springboard ledge. Then Treguard could say - 'well you might not get the sword but you might get the silver for that effort'

The transporter pads towards the end of level 3 in series 4.
"Always step back into the middle or your journey may be longer than you intended...."
Always sounded so sinister, I always wanted to see what would happen if they didn't step back to the middle.

A death by one of the early (Olgarth/Granitas) wall monsters would be interesting. In actuality, they'd probably just say something like "None is the score, so I feed on you!" and have the lifeforce appear, but I've always liked the idea of laser beams shooting out the monster's eyes, frying the dungeoneer to a crisp...

Personally, I'd hope for a more literal and visual interpretation of "Feed on you", maybe involving a giant tongue coming out of their moth and them consuming the dungeoneer... But it's a death I'd have liked to see.

Igneous should have eaten Akash, by all rights. Although he'd have been more of a mid-morning snack than a whole meal.

Someone dying in the first room!
A supremely brainless Series 3 team enters the first room and proceeds to put the die in the knapsack. Not only does the resulting indigestion cause severe Life Force damage, but the shifting of the die in the knapsack summons the giant version, which smacks into the dungeoneer and ends the quest.
Treguard: "Ooh, nasty. Your quest had hardly begun, team, and you still found a way to dice with danger. Hardly a case of never say die. And now I know what I must say. Spellcasting..."

Someone dying in the first room!
By right, Alastair of Series 4 Team 2 SHOULD have perished when he jumped off the spindizzy in the quest choice chamber!
It's even blatantly obvious to see if you go and have a look at the following footage at 3:56 :

Team 7 of Series 5 buying and spellcasting SWITCH instead of CHANGE, only for it to switch dungeoneer Christopher with the ice-encased firestone.

Discounting the "Edward Gorey rooms" I imagined in a previous post, and discounting that message on another forum which I can't mention here...
I'd like to have seen the wall monster feed on someone, seen the barely visible monster at the end of the corridor in Series 1 kill somebody, seen somebody take the Fire Exit, or seen one of the early "stalkers" (esp. Automatum or Behemoth) actually kill.
